No out-of-pocket expenses for those who qualify for the Medicaid CDPAP program
MONSEY, N.Y. - Rezul -- Upstate New York based Horizon Home Care Services announced expansion of their Consumer Driven Personal Assistance Program (CDPAP) coverage areas with a new office in Canton. In addition to this new location in St. Lawrence County, Horizon continues to provide services throughout New York, including Suffolk, Rockland, and Orange Counties; New York City, Rochester, Troy, and Syracuse. CDPAP is a Medicaid program that pays for in-home help provided by a trusted friend or family member..
As a Medicaid-approved and locally-owned Upstate New York home care agency, Horizon is dedicated to ensuring elderly, ill, and disabled New Yorkers receive much-needed in-home care. Their knowledgeable team members guide patients and caregivers step by step, making the qualification and sign up process simple and fast. If qualified, Medicaid will pay the caregiver of choice to come into the client's home.

Benefits of CDPAP include
With a rapidly aging population and more people wanting to stay in their homes, the demand for quality homecare has never been higher. This strong demand results in a tight labor market for caregivers and rising rates. For most Medicaid patients, paying for care out of pocket is beyond their financial means.
Studies show that when seniors are able to remain in their home they're healthier and happier, have fewer falls, and experience lower hospitalization rates than people in a care facility. Unlike long-term care facilities which offer generalized care, an at-home caregiver provides personalized care.
At-home help includes activities of daily living (ADLs) such as bathing, grooming and dressing. Caregivers assist with preparing meals, shopping, laundry, and other housekeeping. Clients receive assistance with getting around and running errands. Additionally, the caregiver of choice monitors their loved one for safety and health concerns, as well as ensuring they receive their medications and attend doctor's appointments. Caregivers are paid for their valuable work.

Horizon Home Care makes the process easy: Once a caregiver is selected, a New York Independent Assessor (NYIA) determines how many hours of coverage the Medicaid recipient can get each week. The client's insurance plan nurse will visit the home to determine eligibility for home care. Horizon quickly handles all paperwork for the caregiver and ensures weekly payment.
Horizon's mission is to make it easy for New Yorkers on Medicaid to get the help they need in their homes, so they can continue living independently at home.
Prospective clients are encouraged to call 315-238-5229 or email Horizon at info@horizoncares.com.
